1. The Scriptures are true, authoritative, sufficient and inerrant.

I hereby give my permission for Lamar Baptist Church to obtain information relating to my criminal history through any law enforcement agency, department or organization of any city, county, state or federal government or any person or organization having knowledge of my identity. The criminal history information, as received from the reporting organizations or persons, may include arrest and conviction data, as well as plea bargains and deferred adjudication. I understand that this information will be used, in part, to determine my eligibility for an employment/volunteer position with this organization, or if already employed my continued employment in a compensated or volunteer capacity. I also understand that as long as I remain an employee or volunteer with Lamar Baptist Church that a criminal history records check may be repeated at any time. I hereby authorize Lamar Baptist Church or authorized representative bearing this release or copy thereof to conduct an appropriate check including, but not limited to records, personal interviews, memoranda, reports of other documents, worker’s compensation records (ONLY AFTER A CONDITIONAL JOB OFFER HAS BEEN MADE), court documents, credit reports, driving records, for verification. I understand that if employment is denied because of information obtained by my prospective employer from a credit bureau, I will be notified and that I am entitled to the name of the report from the credit bureau. I understand that I will have an opportunity to review the obtained information. If I dispute the record as received, I will be given the opportunity to clarify, in writing, and information contained in the report. I hereby further authorize and acknowledge that a photocopy or fax of this authorization may be considered as valid as an original.

I, the undersigned, do, for myself, my heirs, executors and administrators hereby release and forever discharge and hold harmless and agree to indemnify Lamar Baptist Church and each of its officers, directors, employees and agents from and against any and all causes of actions, suits, liabilities, costs, debts and sums of money, claims and demands whatsoever, and any and all related attorneys’ fees, court cost, and other expenses resulting from the investigation authorized by me herein.
